INDIVIDUAL ATHLETE WAIVER
This form may be signed and used for UC San Diego students who are trying out for a particular sports club. It is only valid for up to one week of participation with any sports club’s team. It is NOT valid for any official sports club competitions. (Two weeks after a team’s official start date (tryouts, rookie clinic, practice, etc.), all athletes must be signed up on the Sports Clubs Online Management System).
This form is used as a group waiver for all visiting teams who compete in any of our events/tournaments. All visiting team members must Print, Sign, and Date this waiver before any participation. This form is to be used to pay for any tournament entry fees, equipment, etc. in advance (Due three weeks in advance) OR for reimbursement on non-travel expenses: equipment, repairs, etc. Receipts, invoices, bank/credit statements, and/or other documentation must be attached.
This form is to be used for reimbursement on travel expenses: gas, lodging fees, rental vehicles, etc. Receipts, invoices, bank/credit statements, a trip roster, and/or other documentation must be attached.[All travelers wanting to be reimbursed from a trip must submit their forms and receipts to the SC Office at the same time. Travel documents must be submitted within TEN days following a trip. Tape all receipts onto a sheet(s) of 8 ½ x 11 white paper (do not overlap receipts)]. All students must fill out and sign the second sheet.
APPROVED SPORTS CLUB LOGOS
This form is to be used for any injuries sustained during any sports clubs activity at both home and away locations. It should be detailed, legible, and as complete as possible. These are to be turned in to the SC Athletic Trainers.
This form is used for any significant incidents that happen during a sanctioned sports clubs event. This includes but is not limited to things such as fights, theft, property damage, etc. This form needs to be detailed, legible, and as complete as possible and submitted to the SC Office within 24 hours of the incident.
This form is used to track and verify your team’s community service involvement. It needs to be filled out, signed by an organization contact, and brought into your Sports Clubs director after your team participates. (Please remember community service proposals need to be done before the event and these forms are submitted. Proposals can be submitted via email at least 1 week in advance and must be approved before a team participates).
Use this document if you are getting your physical from a doctor of your choosing. Please note that it must be fully complete and signed by an MD or DO. Physician’s Assistants (PA) or Chiropractors (DC) are not acceptable. Once completed, upload the Physical Examination Form to DSE under the medical documents tab.
This form is to be used if you need an eligibility check done for league participation or an event, but your National Governing Body or Collegiate League does not provide one for you. This form is to be completely filled out and brought to your Sports Clubs director along with your conferences rules and descriptions of student’s eligibility requirements (Due at least 1 week in advance).
This form can be used as an official receipt for entry fees of tournaments you host. If a team is requesting a receipt, please use this form to ensure they receive valid back-up documentation of payment.
This sheet has the updated costs associated with renting University vehicles. These prices do not include gas. To rent a University vehicle, email your Sports Clubs director the type and number of vehicle(s), date and estimated time of pick-up and drop-off, and destination of trip (Due 2-3 weeks in advance).
This form is to be completed and handed in (ORIGINALS ONLY - no scan or fax) by all Sports Club athletes that will be driving teammates to and from approved sports club events (tournaments, games, etc.) in their own personal vehicles. Along with this form, all personal vehicle drivers must send in a copy of their driver’s license and proof of current automobile insurance.
COACH'S CONTRACT This form is to be completed, initialed, and signed by all PAID and VOLUNTEER coaches on an annual basis. It must also be signed by the team president and one of the program directors. This form starts the hiring process. Once completed the original must be submitted to the SC Office. Scans can be used to start the process, but the original must be submitted before a coach can start.
