Community Advisor Signature Page (PDF)
Community advisors sign and return this page to the Center for Student Involvement.

UCSD Group Waiver (PDF)

Waivers for Recreation Clubs who are having other schools/teams compete against them on UCSD facilities. Each opposing team must complete a group waiver. All completed group waivers must be brought to the Recreation Club Advisor after each competition.

Recreation Club Roster (PDF)

Each club must keep an updated roster on file with the Rec Club Advisor.  If the roster is not kept current you will lose the use of a facility.

Recreation Club Waiver (PDF)

This waiver is to be completely filled out and signed by every participant of your Recreation Club before any activity takes place. One waiver per participant per year is required. All waivers must be turned in to the Rec Club Advisor by the thrid Friday of October. Any additional completed waivers need to be turned in upon completion. 

Travel Form (Microsoft Word)

All Recreaion Clubs planning on performing off campus must complete this form and turn it in to the Rec Club Advisor one week before event for approval. No Rec Club travel may take place without an approved travel form.
